Joining Evoque 24 is easy; it may begin with a home visit from our care manager who will discuss the type of care provision that will suit you best.

A care plan is the most important document for your care. We will assess areas of risk and identify your needs; this will be recorded in your care plan. We will then discuss with you and decide how our staff can properly meet your needs, and this will form the basis of your plan of care.

The care plan will continuously be reviewed because people’s needs do change, sometimes on a daily basis, and we have to respond to these changes making sure that we are delivering the right care. Your opinions are important, therefore you will be asked for your input when developing the Care Plan, be it making changes or amendments to it, you will be consulted to ensure that you are fully satisfied with the care you will receive from us,

We also recognise the value of family members, relatives and friends, with your permission we will engage them to participate in the care planning process.
